Coastal Bend Chrysalis Community Board of Directors makes available to you the following forms for download:

IMPORTANT SPONSOR NOTICE:
We can only take 36 candidates on each of our weekend events. This will be by a "FIRST RECEIVED, FIRST REGISTERED" process. If you have questions to this limitation, please feel free to email the Community Lay Director. All Registration is to be sent to the Emmaus Office up until 6 weeks prior to each event, after that cutoff date, please email the Lay Directors of the individual events about where to send registration forms. Lay Director contact information is posted on the
Community Event Page.

If you have any questions regarding registration of a candidate for an upcoming event, please email the Coastal Bend Chrysalis Community Lay Director. She will be more than happy to answer any questions regarding the registration process.

THE CURRENT COST OF EVENT PARTICIPATION IS $160.00 PER PERSON

Chrysalis Flight Application

(High School Ages)

Candidates MUST have completed their Freshman year of HS, AND be 15 years of age.  
Chrysalis Journey Application

(Young Adult ages 19 - 24)

Candidates MUST have completed their High School years or be 19 years of age.
Team Experience Sheets  All persons interested in serving on a Chrysalis weekend should complete a team experience sheet.
